Each reflection form contains three distinct parts. The first portion of the form asks students to summarize the lesson, assignment, or their daily, weekly, or monthly performance. The second portion asks students to rate their performance on a scale of 1 to 5. The last portion of the form really focuses on a growth mindset because students reflect on how they can improve and grow.

This SMARTePlans resource includes a link to a Google Slides document.
